CASE MANAGERS | RESIDENTIAL ASSISTANT

HIV Medical Case Managers (LSW)


POSITION DESCRIPTION
This position provides medical case management services for individuals and families living with HIV/AIDS.  Medical Case Managers must adhere to all standards of social work practice, educate those living with HIV about the disease, facilitate access to medical care and medications, and provide psychosocial support and referrals for a broad range of all services. 

QUALIFICATIONS
Minimum of a Bachelor’s Degree in Social Work or related field, LSW, with two years’ experience. Demonstrates sensitivity to people living with chronic disease and/or experiencing homelessness. Must possess excellent skills in interviewing, assessment, service planning, service delivery and program evaluation.

Knowledge about HIV/AIDS service networks, entitlement programs and other community services. Proven ability to work successfully with staff, clients, their families and support systems, other disciplines and community agencies. Must possess excellent verbal and written communication skills with basic computer abilities. Must have transportation.  

EMPLOYMENT TYPE
Full-time hourly  

SALARY
Full-time hourly: $19.50–$20.70/hr. Full benefits include vacation, health, vision and dental insurance, 401k, licensure supervision and CEUs.

Residential Assistant (Part-time)


POSITION DESCRIPTION
The Residential Assistant, Supportive Housing Program ensures the safety of tenants and of property and helps tenants move forward with healthy living goals. This position is key in providing a safe, structured and secure community environment where people living with HIV and substance use disorders can progress in their lives. Two individuals will be hired.

QUALIFICATIONS
High school diploma or equivalent, basic computer skills for email and documentation. Basic knowledge about HIV/AIDS and the needs of people experiencing homelessness living with co-occurring, chronic illnesses. Effective communication skills as well as the ability to work autonomously. Able to respond to crises in a professional manner.  

EMPLOYMENT TYPE
Part-time hourly; 12–18 hours/week; evening, overnight and weekend shifts ONLY

SALARY
Part-time hourly: $14.80/hr
